Calcium plate fracture is a mechanism of Intravascular Lithotripsy (IVL). Using IVUS, POBA studies from the early 1990’s demonstrated separation between calcium – noncalcium junction (i.e., “debonding”). Importance debonding has not been evaluated in modern era IVL. We hypothesize that significantly contributes to improved compliance IVL treated calcified vessels and evaluate contribution intimal tears. 20 human coronary arteries were subjected laser A Holmium (2J, 5Hz) with 400μm fiber was utilized. Ultrasonic pressure waves at 62±0.4 atm delivered circumferentially. OCT pullbacks recorded pre- post-IVL arterial measured. Mechanisms for using histopathology, microCT, scanning electron microscopy (SEM) finite element models (FEM). Lumen area increased 2.3±0.4 4.3±0.8 mm2, p<0.01. Mean by 2.3±0.6 atm/mL, p<0.05. Post-IVL modifications including fractures, tears identified 73% OCT, microCT SEM. FEM simulations suggest fractures contributed vessel compliance, changes correlate number fractures. also de-bonding greater improvement than alone.
